Fill out appropriate forms on your computer and save or print the documents and fill out by hand.

Make sure to add enough postage. You will need as least $.59
postage if your application forms exceed 7 pages.
Upon receipt of the completed forms, The Maitri Project will contact you by email to notify you that the application has arrived.
The application acceptance process takes 5-7 business days.
Notification of subsidy awarded will be sent by email with an attached contract.
If you agree to the terms of the contract, please sign and return the contract to the Maitri Project.
Subsidized therapy through The Maitri Project will begin only once The Maitri Project has received your signed contract.
